By checking the Terms & Conditions box during checkout on any Driven Dragons Programs, you agree to be bound by all the terms outlined on this Terms & Conditions page.
Refund Waiver & Tuition Insurance Option:
All program fees are non-refundable, including in the event of suspension or removal for non-compliance with the Player & Parent Handbook.
While tuition insurance is not required, we strongly recommend that you consider purchasing it. This coverage can help protect your investment against unforeseen circumstances such as job relocation, serious illness, or injury that may prevent your athlete from completing the season. Given that all club tuition is non-refundable, securing this optional protection is in your best interest.

Photo/Video Waiver:
By clicking on Terms & Agreements, I, the parent/guardian of of my child, acknowledge and agree to the following:
Photo and Video Capture: I understand that during participation at Driven Dragons, my athlete may be photographed and/or videotaped. I acknowledge that these photos and/or videos may be used by Driven Dragons for the purpose of evaluating my athlete's performance.
Social Media Usage: I grant Driven Dragons Volleyball Academy permission to use photographs and/or videos of my athlete on their social media platforms. I understand that my athlete's name will not be used in conjunction with these images or videos. No Compensation: I understand that I will not receive any compensation for the use of these photographs and/or videos.
Release of Liability: I release Driven Dragons Volleyball Academy, its officers, employees, and volunteers from any and all liability arising from the use of these photographs and/or videos as described above.
Revocation: I understand that I may revoke this permission in writing at any time by contacting Driven Dragons Volleyball Academy via email at PeopleSuccess@DrivenDragonsVBA.com. However, I understand that any photos or videos already published may not be able to be removed. I have read and fully understand this waiver and release. I am acknowledging this waiver voluntarily by clicking Terms & Conditions on the purchase of the said program.
Medical Release Waiver:
I, (Parent/Guardian), certify that my child is in good physical condition, is fit to participate in the Driven Dragons Program, and suffers from no physical impairment that might be aggravated by said participation. I certify that my child is covered by medical insurance and that I will not hold Drive Dragons Volleyball Academy or its representatives responsible for any injuries or harm to my child that occurs during participation in the program.
Conduct Behavior Waiver:
We, (Players & Parents/Guardians) acknowledge that PHYSICAL VIOLENCE, FOUL AND ABUSIVE LANGUAGE, HARASSMENT OF PLAYERS, COACHES, OTHER PARENTS OR GAME OFFICIALS WILL NOT BE TOLERATED. Such conduct will be reported by the game official or coach to Driven Dragons. Any incident of a player or coach involving physical violence, assault, or abuse directed against a referee or linesman will be subject to review by the Driven Dragons Leadership Board. The Driven Dragons Leadership Board reserves the right to expel any player, parent, or coach from the organization as a result of misconduct. The use of noise making or applying devices is PROHIBITED.
Concussion Waiver:
I certify that I have clicked on the following link and read the CDC Concussion Fact Sheet for Parents and have a basic understanding of the risks, symptoms, and treatment for concussions.
Injury/Physical Waiver:
Recognizing the possibility of injury associated with volleyball, I hereby release, discharge, and/or otherwise indemnify the Driven Dragons Volleyball Academy against any claim by or on behalf of the registrant as a result of the registrant's participation in the Driven Dragons Programs. My child has received a physical examination by a physician and has been found physically capable of participating in the Volleyball Program.
